?Assessing fitness-to-practice of overseas-trained health practitioners by Australian registration & accreditation bodies (BMC Medical Education)? Tweet

Background: Assessment of fitness-to-practice of health professionals trained overseas and who wish to practice in Australia is undertaken by a range of organisations. These organisations conduct assessments using a range of methods. However there is very little published about how these organisations conduct their assessments. The purpose of the current paper is to investigate the methods of assessment used by these organisations and the issues associated with conducting these assessments. Methods: A series of semi-structured interviews was undertaken with a variety of organisations who undertake assessments of overseas-trained health professionals who wish to practice in Australia. Content analysis of the interviews was used to identify themes and patterns. Results: Four themes were generated from the content analysis of the interviews: (1) assessing; (2) process; (3) examiners; and (4) cost-efficiency. The themes were interconnected and each theme also had a number of sub-themes. Conclusions: The organisations who participated in the present study used a range of assessment methods to assess overseas trained health professionals. These organisations also highlighted a number of issues, particularly related to examiners and process issues, pre- and post-assessment. Organisations demonstrated an appreciation for ongoing review of their assessment processes and incorporating evidence from the literature to inform their processes and assessment development.

'Dance Moms' star: Get in shape, Honey Boo Boo

"Dance Moms" instructor Abby Lee Miller is known for her tough-talking ways when it comes to dealing with young dancers on her Lifetime show, but she also has harsh words for another kid. When asked about TLC's rising reality star Alana "Honey Boo Boo" Thompson, Miller didn't hold back.

"She needs to get in shape," Miller said of the 7-year-old during an interview with TMZ. "She needs to be at a dance studio; she need to be training; she needs to work on her turnout -- the whole nine yards."

Miller hasn't been the only one to remark on the tyke's physique lately. On the season finale of "Here Comes Honey Boo Boo," Alana herself worried that she was too "chunky" to fit into her latest pageant outfit. And "The View's" Joy Behar recently said that she believed the youngster would "grow up to be a big fat woman."

That's an awful lot of commentary about one kid's weight. But in the case of Miller, it seems to come with some degree of empathy. After all, she admits she's not so happy with her own shape either.

"I'm a little frazzled because I was watching the reunion of 'Dance Moms,' and I didn't like my hair and I didn't like my two chins," she told TMZ.

Controversial bill that prohibits regulation of VoIP services signed by

Governor Jerry Brown today signed SB 1161 that prohibits the Public Utilities Commission or any other state agency to regulate Voice over Internet Protocol (VoIP) or Internet Protocol-enabled service providers.

Senate Bill 1161 was authored by Senator Alex Padilla (D-Pacoima).

?This bill encourages the continued growth of these (VoIP) and other innovative services that have become a hallmark of our state,? Brown said in a signing statement addressed to members of the California State Senate. ?The existing consumer protections safeguarded by this bill are crucial.?

Current federal law states that while ?telecommunications service? like landline telephones are subject to regulation regarding terms of service, rates, and access to the 911 emergency line, the Federal Communications Commission (FCC) has not regulated ?information service? like the Internet.

On a state level, telecommunications services are regulated by PUC. Neither PUC nor the FCC has stated whether VoIP services are considered telecommunication or information services and has declined to regulate it the way it does telephone companies.

SB 1161 states that without authorization by statute, either by the state or by federal law, PUC cannot impose regulations on VoIP services. SB 1161 does maintain regulations requiring the maintenance of the 911 emergency line, the maintenance of backup power systems, and allowing enforcement of state and federal consumer protection laws.

Supporters of the bill include TechAmerica, AT&T, the California Chamber of Commerce, and the Voice on the Net Coalition.

Groups opposing the bill include the American Association of Retired Persons, the American Civil Liberties Union of California, the Town of Fairfax, and the Utility Workers Union of America.

(AP) ? A man fatally shot himself in the head Friday on live national television at the end of a high-speed carjacking chase that began in Phoenix and ended about 90 minutes later within 80 miles of the California border.

Fox News was covering the chase that began about midday using a live helicopter shot from Phoenix affiliate KSAZ-TV. The man driving a copper-colored four-door sedan stopped, ran into the desert and placed a handgun to his head and fired.

The man was declared dead at the scene and hasn't yet been identified, according to Sgt. Tommy Thompson, a Phoenix police spokesman.

Fox News anchor Shepard Smith told viewers that the video was supposed to be on a 10-second delay so it could be cut off from airing if something went awry.

"We really messed up, and we're all very sorry," Smith said.

Fox apologized for showing the violence on air.

"We took every precaution to avoid any such live incident by putting the helicopter pictures on a five second delay," said Michael Clemente, executive vice president of news editorial. "Unfortunately, this mistake was the result of a severe human error and we apologize for what viewers ultimately saw on the screen."

More frequently than its rivals, Fox News Channel picks up car chases from its local affiliates and airs them live. It's gripping television, a live mystery with no clear resolution, and often provides a short-term ratings boost as viewers tune in to see how it ends. Critics say the chases themselves rarely rise to the level of national news. The Phoenix station was not airing the chase live when it ended.

Thompson said the man allegedly stole a car from a couple at gunpoint outside a Phoenix restaurant just before 11 a.m. MST.

Police officers located the vehicle and began a pursuit and the man fired several shots, Thompson said. He said the shots hit the police vehicle and the officers escaped injury.

The suspect headed west on Interstate 10, then pulled onto a dirt road that Thompson said was "70 to 80 miles east of the California line."

"He got out of the car and shot himself," Thompson said. "Efforts to revive him were not successful and he was dead at the scene. We don't have an ID yet."

Fox returned repeatedly to shots showing the car passing big-rig trucks that typically travel at about 70 mph as if they were standing still.

Police cars did not appear to be immediately behind the car during most of the chase.

Shepard Smith was narrating the video and clearly had his doubts about what was being shown from the moment the man stopped the car. "This scares me," he said.

"You wait for the end of these things and you worry about how they may end up," he said. "This makes me a little nervous, I got to tell you. A little nervous."

The video showed the man running erratically in a field before putting the gun to his head and firing. He fell to the ground.

Fox's picture quickly cut to Smith, who was shouting "get off, get off, get off, get off."

After the commercial break, Smith apologized repeatedly.

"That didn't belong on TV. We took every precaution we knew how to take to keep that from being on TV and I personally apologize to you that that happened," he said.

Zamzee: Activity Monitor And Interactive Gaming Site Boost Physical ...

Who knew a digital game could motivate kids to exercise?

A new study sponsored by the nonprofit HopeLab and the Robert Wood Johnson Foundation shows that kids who use an activity meter called a Zamzee and its corresponding game website over six months are 59 percent more active than those who don't.

The Zamzee, created by HopeLab, is a small device that works by monitoring movement and physical activity. That exercise information is then loaded onto a corresponding game website, where kids can go on to see how much they moved and earn points and rewards. "This study shows that technology is not just part of the problem; it can also be part of the solution in helping kids be more physically active," Steve Cole, Ph.D., the Vice President of Research and Development at HopeLab and a professor of medicine at the University of California, Los Angeles, said in a statement.

The study, presented at the 2012 Obesity Society Annual Meeting, included 448 kids of middle-school age around the United States. Half of them were assigned to use the Zamzee and the corresponding game website for six months, while the other half used just the Zamzee but didn't have access to the website.

Researchers found that the kids who used the Zamzee and the website completed 59 percent more exercise each week than those who didn't have access to the motivational gaming website. Specifically, those who used Zamzee and the website exercised about 118 minutes each week, while those who just used the Zamzee exercised about 74 minutes each week, Scientific American reported.

Plus, kids who were overweight exercised 27 percent more and girls exercised 103 percent more when they used the Zamzee and the website, researchers found.

All of this extra exercise translated into actual effects on the body, too -- use of the Zamzee was linked with decreases in "bad" LDL cholesterol levels in the kids, as well as better blood sugar control, the researchers found.

According to the Centers for Disease Control and Prevention, kids should be getting at least an hour of aerobic exercise (like running) every day, as well as at least three days a week of muscle-strengthening exercises (like push-ups) and bone-strengthening exercises (like jumprope).

In this case, Columbia -- or, more specifically, Don Ienner -- believed in this record, so much so, that the label was willing to underwrite fans' investment. Well, for a limited time, until September 30, 2001, that is. If you didn't notice, there was an expiration date on that shit. You bet there was. Ienner was no dummy.

According to the band's recent Behind the Music episode, Ienner recognized the song's potential immediately after hearing just a rough demo. He must've also anticipated that by fall, six months after the record was released, radio would play the living hell out of the album's title track until everybody in the country was literally sick to death of hearing Train, and would subsequently demand all of their money back, along with reparations for any associated pain and suffering.

So there you have it, the simple solution to saving the music industry.

SACRAMENTO (CBS / AP) ? Campbell Soup Co. is closing two U.S. plants and cutting more than 700 jobs as it looks to trim costs amid declining consumption of its canned soups.

The world?s largest soup maker said Thursday that it will close a plant in Sacramento that has about 700 full-time workers. The plant, which makes soups, sauces and beverages, was built in 1947 and is the company?s oldest in the country. That also means it has the highest production costs of Campbell?s four U.S. soup plants.

Campbell also plans to shutter a spice plant in South Plainfield, N.J. that has 27 employees. Production will be shifted to the company?s other spice plant in Milwaukee.

Employees at the two closing plants were notified that there would be a meeting at 6 a.m. local time Thursday; about 400 workers showed up in California, where they were told of the closure.

?It?s always difficult, even when there?s a business case that is compelling,? said Anthony Sanzio, a Campbell spokesman. ?You?re dealing with people, and this is going to impact 700 employees who?ve worked together closely for many years.?

CEO Denise Morrison, who was been on the job for about a year, was not present at the meeting.

The announcement comes as Campbell looks to freshen up its image with dozens of new soup flavors and sauces intended to lure younger consumers. Many of the new products come in pouches designed to convey a fresher feel, rather than the iconic steel cans that have long defined the company.

Those new pouches are manufactured with another party and are not made at Campbell?s soup plants.

Although Campbell makes other products such Pepperidge Farm baked goods and V8 vegetable juices, soups account for half its revenue. And the Camden, N.J.-based company has struggled to fix the soup unit.

Over the past decade, overall canned soup consumption is down 13 percent, according to the research firm Euromonitor International, as fresh soups have become more widely available at supermarkets and restaurants. Campbell?s share of the market has also declined to 53 percent, down from 67 percent a decade earlier.

To expand into products with more growth potential, Campbell this summer purchased Bolthouse Farms in a $1.55 billion cash deal. The company says the move will help it stake a claim in the fresh packaged food category, which is growing at a faster clip than the broader packaged food market.

Campbell Soup has about 19,900 employees globally. The company said the California plant is expected to close by July 2013 and the New Jersey plant by March 2013.

The company expects the closings to result in about $115 million in pre-tax costs. Its actions will also require about $27 million in capital spending.

Annual savings are predicted to be about $30 million starting in fiscal 2016.

Shares of Campbell fell 11 cents to close at $34.75 Thursday.

3-D images of 300-million-year-old insects revealed

ScienceDaily (Sep. 25, 2012) ? Stunning 3-D images of 300-million-year-old insects have been revealed for the first time by University of Manchester researchers.

Writing in the journal PLOS ONE, the scientists have used a high resolution form of CT scanning to reconstruct two 305-million-year-old juvenile insects. Without the pioneering approach to imaging, these tiny insects -- which are three-dimensional holes in a rock -- would have been impossible to study.

By placing the fossils in a CT scanner, and taking over 3,000 X-rays from different angles, the scientists were able to create 2,000 slices showing the fossil in cross section.

From these slices the researchers created 3D digital reconstructions of the fossils. This process allows them to learn more about the lifestyle, biology and diet of the insects, one of which is similar to a modern day cockroach, and glimpse fascinating insights about how both were adapted for survival.

One of the insects reconstructed by the scientists is characterised by a large number of sharp spines. It is a new species and genus which does not exist today.

The other is an early predecessor of one of the great survivors of the insect world, the cockroach, and is one of the best preserved examples of this age ever seen by insect palaeontologists. Researchers suspect from its well preserved mouthparts that it survived by eating rotting litter from the forest floor.

Both are members of a group called the Polyneoptera -- which includes roaches, mantises, crickets, grasshoppers and earwigs. But analysing the exact relationships of the insects will be difficult for the researchers, led by Dr Russell Garwood of the University of Manchester's School of Materials, as insects have a habit of dramatically changing appearance as they develop.

Dr Garwood said: "The most dramatic change is seen in insects like butterflies, which change from a larva, to chrysalis, to adult. But relatively few people look to the fossils try and work out how such a life cycle may have evolved.

"We are hoping that work like this will allow us to better understand the biology and development of these early insects, and how major innovations may have come about.

"Around this time a number of early 'amphibians' were insectivores -- they lived by eating a lot of insects. The spiney creature was a sitting duck, as it couldn't fly, so the spines probably made it less palatable. It is bizarre -- as far as we're aware, quite unlike any members of the Polyneoptera alive today."

"The roach nymph is much like modern day cockroaches -- although it isn't a 'true' cockroach, as it may well predate the split between true cockroaches and their sister group, the mantises."

"This is very much a first step, and I'll be spending the next few years looking at other fossil insects to build on this work.

Professor Philip Withers, co-author on the paper, added: "I am very excited by our fossil work which is providing unique information in 3D."

What this shows us is that Google is devoted to updating its own apps to work on the Intel platform, and hopefully now other app developers will follow suit with their own updates.

'Fringe' to end with a 'love letter' to fans

LOS ANGELES --?"Fringe" fans may be feeling bittersweet as the sci-fi TV show enters its final season, but executive producer J.H. Wyman pledged on Wednesday to give fans a satisfying conclusion and "the love letter they deserve."?

"Fringe" -- ?co-created by J.J. Abrams of "Lost" and "Alias" fame, Alex Kurtzman and Roberto Orci -- follows an FBI team who use methods outside of mainstream science to solve mysteries relating to a parallel universe.

The fifth and final season, which starts on Friday on Fox, will pick up from the "Letters in Transit" episode in April 2011 that threw the series into year 2036, where the original Fringe team is freed from encasement in amber and must face a dystopian society.

Wyman, also a writer on "Fringe," told reporters in a conference call on Wednesday that he "fell in love" with setting the story in the future, and wanted to focus the final season on the show's characters.

Peter Bishop (played by Joshua Jackson) and Olivia Dunham (Anna Torv) provide the main love story of the series as their relationship struggles in the face of time-bending obstacles.

"No love story worth telling is an easy love story. It's the hills and the valleys that make a relationship, in my opinion, really dynamic and worth watching and the harder the tale, the more worthy the pay-off," Wyman said.

While he declined to reveal details of upcoming plots, Wyman said he hoped fans would "enjoy the surprises," and that the ending would satisfy audiences and give the fans "the love letter they deserve."

"I want to feel that my characters evolved in a place that they deserved, sometimes maybe unexpectedly, but logically they've come to a conclusion that makes me feel satisfied," Wyman said.

"I want to tell these real odyssey stories about these people, and give them a little more sense of continuity," he added.

"Fringe," which combines science fiction with elements of police procedural drama, debuted on Fox in 2008 averaging 10 million viewers in its first season. It became a cult favorite, but ratings slipped last season to about 4.2 million viewers.

Many single stars may have been born as two separate suns, which merged into one during the first million years of their life. That is the unexpected finding from a computer simulation of binary stars in a young star cluster.

Double stars are common. For example, Sirius, the brightest star visible in the night sky, consists of two stars that orbit each other every 50 years.

Studies of star-forming regions indicate that binary stars start off with a uniform distribution of orbital periods: the number of binaries younger than a few million years that orbit each other every 1?10 years, 10?100 years, 100?1,000 years, and so on, is about the same

Older binaries show a different pattern. The most common orbital period for double stars outside of star-forming regions is about 200 years, and both shorter and longer periods are rarer.

The scarcity of binaries with longer orbital periods is easy to explain: to have a long orbital period, the stars must be quite far apart. For two stars with a total mass adding up to that of the Sun, a 200-year orbital period would put them about as far from each other as the Sun is from Neptune or Pluto. Stars at much greater distances feel little of each other's gravity, so the gravitational tug of a passing star can disrupt the system and break it apart.

Inward spiral
But where did the short-period binaries go? In a paper in Astronomy and Astrophysics, three astronomers in Germany report how they simulated a star cluster resembling that in the Orion Nebula, the best-known stellar nursery. Their model assumes that as two stars close together in a binary system whirl around each other, they interact with the gas in the cluster. The interactions can modify the stars' orbits, causing them to spiral towards each other until they merge and become a single star.

"It was quite astonishing," says Thomas Kaczmarek, an astronomer at the Max Planck Institute for Radio Astronomy in Bonn, Germany, and a co-author of the paper. "We just took the parameters that we needed from this cluster, and what came out is that we almost perfectly get this period distribution observed in the field."

Steven Stahler, an astronomer at the University of California, Berkeley, who studies star formation, approves of the work. "It's plausible," he says, but adds that proving the idea correct will be difficult. The same gas and dust that cause the close-in binaries to merge also prevent their visible light from reaching Earth. Astronomers must conduct observations using infrared radiation, which penetrates the dust, but often requires space-based observatories to detect.

In one unique case, astronomers have actually watched two stars in the constellation Scorpius spiral together. The merger sparked a bright flare of light called a red nova, and a similar eruption inside a young star cluster could show that young binaries do indeed merge. However, the Scorpius binary didn't belong to a star cluster, and Stahler thinks that a merger inside a gas-filled cluster might be too gentle to unleash a nova.

Could the Sun have been born as two separate stars that later merged? "It could have happened, but I think it would be a bit far-fetched," says Susanne Pfalzner, an astronomer at the Max Planck Institute for Radio Astronomy and a co-author of the latest paper. She suspects that a merger would have disturbed the newborn Sun's planet-forming disc, preventing the birth of rocky worlds including Earth.

Exposure to school-age children ups severity of cold infections

ScienceDaily (Sep. 26, 2012) ? Exposure to school-age children raises the odds that a person with lung disease who catches a cold will actually suffer symptoms like a runny nose, sore throat and cough, according to a study just published in the Journal of Clinical Virology.

That finding, the result of a study that drew upon a databank of 1,000 samples of sputum and nasal secretions from people with chronic obstructive pulmonary disease or COPD, comes as a surprise, says Ann Falsey, M.D., professor of Medicine at the University of Rochester and an infectious disease expert at Rochester General Hospital. Falsey is senior author of the study, which was led by first author Anurag Gandhi, M.D., an infectious disease fellow at the University of Rochester Medical Center.

"Before we conducted this study, I would have expected other factors, perhaps the severity of underlying disease -- the state of the patient's general health -- to indicate who would actually suffer symptoms from their colds," said Falsey. "Instead, contact with school-age children is the only risk factor we found, and it increases both the risk of infection and also the risk of suffering symptoms once you've caught a cold."

Many studies, including this one, have found that being around school-age children increases the risk of infection, but the new findings go one step further. Of people who come down with colds, the course of the infection is much more likely to be worse in people exposed to children.

In everyday life, many people have no symptoms and don't even realize it when they catch a cold. It comes as no surprise that Gandhi and Falsey found that more than one-third of people infected with a rhinovirus, the bug that causes most colds, had no symptoms.

But further analysis of the data showed that of those people who were infected, people whose infection turned into real-life symptoms -- congestion, runny nose, sore throat, hoarseness and so on -- were about twice as likely to have contact with school-age children as people whose infections did not become symptomatic.

While the study was not designed to tease out a possible explanation, Falsey said it's possibly because colds run a bit more rampant in young children. That's because they haven't had many years to build up immunity to the vast family of rhinoviruses that we encounter repeatedly throughout our lives. Children's colds simply tend to be worse; they have more nasal secretions, for instance, as anyone who has tracked a sick child throughout a household will attest.

"We know that kids shed more virus longer when they get a cold," Falsey said. "It may be that your chance of developing cold symptoms is related to the amount of virus you're exposed to, and if you're around small children, you're exposed to more virus. The idea makes sense, but it's speculation at this point."

To avoid colds, Falsey counsels everyone -- people with emphysema like those in the study, but everyone else as well -- to practice good hand hygiene. That means washing your hands often, especially after you have sneezed or covered your mouth when coughing. Also, avoid touching your eyes and nose as much as possible. And, try to avoid sick children.

"We all know that children are efficient germ-spreading vectors," said Falsey, "and we know that young children oftentimes don't have ideal hygiene habits. It's not unusual for them to accidentally sneeze in your face, for instance. If you have a grandchild who is sick, it may simply be smart to plan a visit for another day."

The study relied upon close monitoring and sampling of 127 people with emphysema who were evaluated six times each during one year. At all visits, nasal secretions were sampled, and sputum samples were obtained when available.

Gandhi and Falsey's analysis of the resulting 1,000 samples of bodily fluids, stored for nearly a decade at minus 112 degrees Fahrenheit, also turned up another unexpected finding. They found that adults who were on home oxygen use were somewhat protected against the added risk posed by schoolchildren. Again, the study was not designed to answer why, since the finding was unexpected. It could be, for instance, that young children were more apt to avoid close contact with adults who were wired up with tubes and apparatus that are scary to many children.

Other authors of the study include Edward Walsh, M.D., professor of Medicine at URMC and an infectious disease expert at RGH; biostatisticians Derick Peterson and Andrea Baran of the University of Rochester Medical Center; and technician Maria Formica and nurses Patricia Hennessey and Mary Criddle at Rochester General Hospital.

This has got to be the icing on the Japanese cake. The otherwise bland website of the Japanese Ministry of Finance, more specifically the FAQ page on government bonds, has been catapulted to stardom on Facebook and Twitter. Not in a good way. As you flip through the MoF?s website, page after page, you will mostly see zero Facebook likes and zero tweets. Social media and the MoF ignore each other.

But go to the FAQ page, skip down past the categories of Budget, Taxation, and Tariffs to item 4, Government bonds. Under the second group, skip past Tax questions for individuals, Miscellaneous (Is it a crime if I make a copy?), Price and yield questions, and Coupons to the infamous question 5: ?In case Japan becomes insolvent, what will happen to government bonds??

The MoF website isn?t some blog to be ignored (at your own risk) but the official voice of the most important ministry of the most indebted country in the world, whose debt will reach 240% of GDP by the end of this fiscal year. The country borrows over 50% of every yen it spends, and it spends more every year. With no solution in sight. Other than more borrowing. Certainly not cutting the budget, which would be too painful. It wouldn?t be enough anyway. Even cutting the budget in half would leave a deficit. And the recently passed consumption tax increase? It will raise the tax from its current 5% to 8% in 2014 and to 10% in 2015, way too little to deal with the gigantic problem, and years too late. Yet it won?t kick in unless GDP grows at least 2% per year?which has practically no chance of happening.

No, there is no longer a good solution. And everyone knows it.

About 95% of Japan?s debt is held within Japan by government-owned institutions, the Bank of Japan, banks, companies, pension funds, and directly or indirectly by individuals. Hence the question??In case Japan becomes insolvent, what will happen to government bonds???is of primordial importance to just about all Japanese adults.

The question and its answer weren?t decided by some underling. Each word was carefully weighed by experts in the highly hierarchical bureaucracy of the MoF. As these words were polished and examined for every nuance, they were passed up the ladder until they landed on the desk of an official at the very top who approved not only the wording, but also whether or not that question should even be on the website. And the official answer is:

??Rest assured that the Japanese government will redeem the bonds responsibly.??

?Rest assured!? How bondholders can possibly rest assured under these circumstances remains a mystery, in particular since the MoF then proceeds to tell them exactly how they will get kicked in the groin: bonds will be redeemed ?responsibly.?

Not when they mature, but responsibly.

Thus, we have the MoF?s official action plan for the moment when the big S hits the fan, the moment when Japan with its declining wages and shrinking working-age population can no longer save enough to mop up all the government bonds necessary to keep the government afloat [read.... Japan?s Slow-Motion Tsunami].

A selective default. Bonds will retain their ?value,? but the government won?t redeem them when they mature. It will redeem them in bits and pieces, stretched into all eternity, as it sees fit.?You?ll die before you?ll see your money.

This is THE answer, the official answer we?ve been looking for all along, and now we find it on the MoF website where it would have remained hidden in plain sight had not some enraged Japanese spread the word via Twitter and Facebook.

And they filled the ether with caustic FB comments:

  • ?This is absolutely nothing but mocking the general public. How dare they say such gibberish!??
  • ?Should be a joke.?
  • ?Isn?t it supposed to go, ?Rest assured that the Japanese people will redeem the bonds responsibly???
  • ?Yeah, the only thing they could rely on is the people?s savings....?

When the legendary savings of the Japanese are drying up, as they?re in the process of doing, the word responsibly will take on a new meaning within the context of one of the greatest recent acts of governmental irresponsibility: creating that debt monster in the first place.
